First Visit Information

All initial visits last one hour in length. The first visit will consist of a patient interview and evaluation of your symptoms.

Please arrive at our facility 10 minutes prior to your scheduled appointment to fill out the required paperwork. You also have the option of printing the required forms, filling them out and bringing them with you to your initial visit.

Wear comfortable clothing so you can move easily. We have changing rooms and showers for your convenience. Parking is free and located in front of our building.

Physical Therapy New Patient Forms

Please print and fill out the Intake Forms and Cancelation Policy Form for your first visit. Review the New Patient General Information to know what to expect from your first visit and if you need a referral form for your Physician to fill out for insurance purposes we have one you may print off to have filled out by your Physician.
